(Xbox 360) Fallout: New Vegas Glitch *Need help!*?

You need to be a little bit clearer about who you are working for. Since you said she I am thinking that you mean NCR's Colonel Moore at Hoover Dam who give you the quest For the Republic Part 2. If not you are either working for Caesar's Legion, Mr. House, or Yes Man. I think that your problem has to do with the Brotherhood of Steel and not your reputation with the NCR. Make sure that you are NOT WEARING T51-b power armor because the NCR will shoot you on sight due to your disguise as a member of the Brotherhood of Steel. You can wear T-45d because it does not pertain to a faction. If your wearing this it is large armor and makes a lot of noise resulting in fail sneak attempts but if you got to Colonel Moore she would attack you too. Make sure you do ALL of the main Brotherhood of Steel quests. I don't understand why there was a BoS Paladin at the dam but that is not normal. I might be going out on a limb here and this is probably false but I think that Rex might have something to do with it. Especially if you gave him Lupa's brain in "Nothin But a Hound Dog" since Lupa was a legion dog (far fetched but this is off the top of my head). After many BoS quest you should come across a point (as you said) to wipe out the BoS. Go talk to Elder McNamara (if you put Hardin in charge it is impossible) and say that the NCR will destroy them unless they sign a truce. You probably don't have to but you should complete Veronica's quests. Veronica can be found at the 188 Rest Stop. If you can not fix it through any of these I would go to older saves (if that's an option), wait close to the Dam for 3 days, or go to Yes Man. Yes Man was put into the game as a failsafe so if you made a situation where you can not complete the game (kill Mr. House, President Kimball, and Caesar) that you could complete the game. I know it might suck (not Yes Man but all your hard work lost) but you can complete the game that way. You can also try sending an E-mail to Bethesda via their website. They also have a glitch and crash support with many common glitches. For now, try these, and if they don't work then do quests and try later, wait for a patch, or go to Yes Man. Try going to the Fallout Wiki too. Just type it in on Google.
